I'm sure you really mean how do SIMS get pregnant. Now that that's cleared up, let me just say--um--it works like it does in real life, kinda. Your sims have to "woohoo" in a bed.

You have to have both your sims relax in a double bed and try for baby. You'll know your pregnant when a lullaby sounds when their done. You can also get pregnant in the hottub, photo booth, and camera booth thing.:)

First of all to clear this up, you can't have a baby on console. So, in order to have a baby, Your sims must be relaxed in a Double Bed, and click Try For A Baby. If you have your volume down, then if you get them to relax again and you click on More and it doesn't show Try For A Baby, then your sim is pregnant. For the sims that are gay and lesbians. Just have your sims deeply in love and the option "Do you want to adopt a baby?" will appear. If you want alein babies, there are 2 ways: One is to Try For A Baby with an alien and another is to get abducted by aleins by using the telescope.

You can do try for a baby in:

The photo booth
The hot tub
The double bed
The changing room
The car

If you want an alien, an easy way is to do boolprop testingcheatsenabled true in the cheatbar. hold shift, click on a sim and do tombstone of L and D. A grave appears. click on it and select Make me alien pregnant, or if you want a normal baby do Get pregnant with... but it won't give you any aspiration if your sim Wants to have a baby.
